Beste patiënt,
Via deze weg wil ik u graag informeren over de AVG-wetgeving, wat dit voor u inhoudt en hoe de praktijk hier aan voldoet.
Zoals u waarschijnlijk weet is de AVG-wet in zijn werking gegaan en voldoet ook Aurora oefentherapie aan de eisen van deze wet. Het betreft de nieuwe Europese wetgeving rondom data en privacy. Dit brengt wat veranderingen voor u en de praktijk mee. Graag licht ik dit kort aan u toe.
Wat betekent dit voor de praktijk?
Aurora oefentherapie werkt conform de nieuwe AVG wetgeving en de daarbij horende veiligheidsstandaarden. Dit houdt in dat de procedures rondom de informatie beveiliging verder aangescherpt zijn. Als patiënt zult u hier weinig van merken omdat dit vooral op praktijkniveau wordt doorgevoerd. Toch vermeld ik dit graag zodat u weet dat uw privacy gewaarborgd wordt. Daarnaast wordt er gewerkt via Winmens, een elektronisch patiëntendossier, die voldoet aan de veiligheidseisen.
Wat betekent dit voor u?
Doordat de AVG wetgeving van kracht is gegaan veranderen uw privacy rechten. U heeft het recht om uw persoonsgegevens in te zien, aan te laten passen of te laten verwijderen. Daarnaast heeft u het recht om uw eventuele toestemming voor de gegevensverwerking in te trekken of bezwaar te maken tegen de verwerking van uw persoonsgegevens door Aurora oefentherapie en heeft u het recht op gegevensoverdraagbaarheid, wat wil zeggen dat u bij ons een verzoek kan indienen om de persoonsgegevens die wij van u beschikken in een computerbestand naar u of een ander, door u genoemde organisatie, te sturen. Ook heeft u het recht om (een deel) van uw gegevens te laten vernietigen.
U kunt uw verzoek tot inzage, correctie, verwijdering, gegevensoverdraging van uw persoonsgegevens of verzoek tot intrekking van uw toestemming of bezwaar op de verwerking van uw persoonsgegevens sturen naar djoekenumannieuwendijk@gmail.com
Verder zal er aan de zorg die verleent wordt niks veranderen. U als patiënt staat nog steeds centraal in de praktijk. Mocht u nog vragen hebben verzoek ik u vriendelijk om contact op te nemen per mail, telefonisch of in de praktijk.

In this way, I would like to inform you about the AVG-legislation, what this means for you and how we comply at the practice.

What does this mean for the practice?
Aurora exercise therapy works in accordance with the new AVG legislation and the associated safety standards. This means that the procedures concerning the information security further tightened. As a patient you won’t notice it very much because this especially on practice level will take effect. Anyway, I like to inform you so you know that your privacy is guaranteed. In addition, we worked through Winmens, an electronic patient record, which complies with the safety requirements.
What does this mean for you?
Because the AVG legislation is in its effect, your privacy rights also changed. You have the right to see your personal information, to make it fit or removed.
